Common Threads After Care Sign Up
This form is for September 2018 - June 2019 After Care sign up. Once we confirm space we will send you an invoice for a deposit (equal to one month's fees). After payment is made your spot will be confirmed. Payment will then be required on the first of each month, preferably by postdated cheque.

If you have multiple kids to apply for, please fill in one form for each kid and add notes at the bottom so we can consider them as a family application, not individuals.

If we have reached capacity, we will contact you to see if you'd like to be on our wait list.
