Instructions for Requesting Sack Lunches

•Complete and submit the form below a minimum of ten (10) business days prior to the event.

•Cost: Student lunches are at no charge, the cost for an adult lunch is $3.55.

•Adjustments to the bag lunch count may be done as late as 12:30 pm the day prior to your event date.

•Complete cancellation of your request, requires a minimum 24 hour email notice prior to the event pick- up date/time.

•Lunches must be picked up at the Central Warehouse at the time indicated on this request.

•A roster listing all students and adults who have signed up to receive a bag lunch must be given to the cafeteria lead the day before the sack lunches are picked up. Payment for adult meals must be made prior to the pick up date. It is the responsibility of the person ordering the lunches to collect payment.

•To be in compliance with ADE guidelines for meal reimbursement, the teacher is responsible to check off each student’s name on a duplicate roster as they receive their bag lunch. This roster must then be signed and dated by the teacher and returned to the cafeteria lead immediately following the class return to school.
