MYS - Spring Season 2020 to Fall Season 2020 COVID-19 Refund/Donation Request Options
Please complete the following form, for each player in your household, by May 15, 2020.
MYS has determined that the default action for all Spring 2020 registration fees will be to apply an account credit for the upcoming Fall 2020 season.
A $15 operating charge will be withheld for each refund requested.

We are requesting a refund of our Spring 2020 fees, minus the $15 operating cost with-holding.
We would like to donate 100% of our Spring 2020 fee to the MYS Financial Aid Program to assist families that would not be able to participate in the Fall 2020 season.
