Spring in my Steps 2020 Waltz Weekend Registration Form
Preregistration is required. Space is limited. Please fill out one form per person. Also, unless you are accomplished in both leading and following and comfortable in switching roles, we must be able to match you with another person dancing complimentary lead/follow role, so we encourage you to specify a partner on your registration form. All leads and follows who do not specify a partner, will be matched with a dancer of complementary role in order of registration. We will notify you by email when your registration is tentatively accepted. To be fully confirmed, we must receive your payment within 7 days of your tentative acceptance.

We provide generous snacks during breaks throughout the weekend, and participants are encouraged to stay on-site during the Saturday lunch break. For your convenience, we will provide an optional catered lunch at an additional cost of $10 per person. (See PAYMENT information below.)

Do you want us to try to find a place for you to stay with a local family? A limited number of home stays are available. You should have a backup plan, such as a hotel reservation, in case we cannot find a place for you. All requests for home stays must be received with this registration form. If you are not requesting a home stay, you can safely jump down to the Liability Waiver. If you are requesting a home stay, please answer all of the housing questions below.

I understand and agree that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. Possible sources of injuries include, but are not limited to, missteps, dizziness, loss of balance, slipping, falling, and colliding with other dancers or objects. In recognition of this, I agree to assume all risks and responsibility for any such injury, which I may have or which may accrue to me during the waltz weekend sponsored by Lexington Vintage Dance (LVD) March 6-8, 2020. Furthermore, I do hereby fully exempt, release and indemnify LVD, its officers, volunteers, and/or instructors and the City of Berea from any and all claims from injuries, damage or loss which I may have or which may accrue to me as a result of my participation in the waltz weekend. I also give LVD permission to use any picture of me at the waltz weekend in or on any form of publicity for LVD or an LVD-sponsored event. Registrations are for one person only. Please fill out a form per person if registering multiple people. If you would like to make a tax deductible donation to LVD, please use the other line and indicate your registration type (student or non-student), whether lunch is desired, and the amount of your donation. If you are requesting the student rate, tell us where you are enrolled.
We will notify you by email when your registration is tentatively accepted. For online payments, we use an online service provider called Stripe which uses most stringent level of certification available in the online payments industry. You may pay with almost any credit or debit card or electronic funds transfer from your bank. Alternatively, you may simply mail us a check via the US Postal Service. With any of these choices, we offer a 7 day grace period for you to pay after receiving notification of your tentative acceptance to the weekend. Hence, your particular payment choice will not alter your acceptance status.

Since combining the payments of registration partners is fine, we need to know the name of the expected payer if it is not the same as the registrant's badge name.. Your registration will be fully confirmed once payment is received.
