Change of Placement
Please use this form to provide information regarding your desire to change your student's academic placement. Someone from FSMS will be in contact with you after you submit your responses, so please feel free to ask any questions that you may have.

This request must be made at least 3 DAYS ahead to give time for schedules to be changed.

**PLEASE KEEP THE FOLLOWING IN MIND AS YOU MAKE YOUR DECISION**

1. A change in placement MAY result in a change of schedule for your student (which also likely means different teachers).
2. It's possible that a change in academic placement MAY result in two grades when report cards come out, one for the in-person portion and one for the virtual portion).
3. A move from VIRTUAL to IN-PERSON will remain in effect until at least the end of the quarter (unless there are extenuating circumstances that can be discussed with school officials).
