The EAC foundation is a non profit organization that is focused on research and funding for pediatric cardiomyopathy.
We would love for you to join us this year at Olympia Spa Country Club Golf Course on May 6, 2023. Registration begins at 11:00, lunch will be provided by 11:15-11:30, and shotgun start at 12pm. The format for the tournament is a 4-man scramble. We will have an after party immediately after golf with live music and heavy hors d'oeuvres; feel free to invite your significant others. In order to reserve your spot, you will need to pay for the invoice within 72 hours. The form alone will not secure your participation. Mulligans and T-Blasters can be purchased the day of the golf tournament.

Silver sponsorship: $500 (this includes all of the above (team, beer, koozie, etc. +sign)
Gold sponsorship: $750 (This includes all of the above + your name/business on koozie, cup, and sign)
Platinum sponsorship: $1000 (This includes all of the above + sponsor on banner)
