INSTRUCTIONS:
The “HEALTHY CHURCH LIFE SURVEY” is a tool to help you gain perspective on the overall health of your ministry. It is nothing more than a simple tool to help you be more SELF-AWARE.
SELF-AWARENESS is an absolute fundamental prerequisite of leadership and is CURATIVE.
Therefore, answer the questions as honestly as you possibly can!
We’ve identified seven areas of focus as it relates to church health. For each area, you’ll find five statements.
For each statement, rate your response on a scale of 1 to 5, with 5 being “strongly agree” and 1 being “strongly disagree.”
Once you’ve rated each of the 35 statements, you’ll receive an email that contains the results to your survey. You’ll find a score for each of the 7 areas, as well as your overall score.
Use these scores to help determine which areas of your church are thriving and where it has an opportunity for growth.
We also encourage you to share this survey with other key leaders in your ministry so that you can compare your results to others on your team.
