Virtual Summer Enrichment Program Interest Survey for NYC Public School Students in Grades 3 - 8
With support from the Schusterman Foundation and the Heckscher Foundation for Children, Practice Makes Perfect is offering a six-week virtual summer enrichment program for children who have just completed grades 3 - 8. Over the course of six weeks, children will engage in 18 instructional sessions with a Practice Makes Perfect Fellow and a small group of peers. Children will also be given one additional hour of self-paced work in math and reading to help with content retention.

The program is FREE for students attending NYC Public Schools. To be considered for a spot you must complete and submit this form by June 29th, 2020. Your child’s enrollment will be confirmed by July 3rd, 2020. Spots are limited!

If you have multiple children you would like to enroll in the Virtual Summer Enrichment program, please include information on all interested children in this form

The program will take place online from July 6th - August 22nd. There will be two session options, with virtual lessons taking place: Monday - Wednesday or Thursday - Saturday.

There will be a one week break from July 27th - August 1st

Filling out this form DOES NOT guarantee a spot in the virtual summer enrichment program. This is simply an interest survey. Our team will be in touch with you to explain the program, answer questions, and review expectations before registering your child for the program.

Students MUST be enrolled in a NYC public school. Practice Makes Perfect reserves the right to remove any student who is not actively enrolled in a NYC public school from its programming at any time.

Practice Makes Perfect's summer program might not satisfy mandated summer school requirements for your child’s school; please check with your child’s principal to ensure you are meeting their requirements.

The information you provide in this survey is private. This information will be used for our internal records, program evaluation purposes, and to let you know about future education opportunities.
