This is the online registration form for the May 2021 Newbury Park International Baccalaureate Programme exams. Please fill this out ONLY if you are planning on taking IB tests during Spring 2021. Note that your responses to the questions below are required by all IB schools testing in the US. We understand that some of the choices (e.g. race, legal gender) are quite limiting and ask that you please fill in this form as accurately as possible. For more information about IB exams, visit the IB page under Academics at nphs.org

To complete your IB registration, follow the two steps listed below:

1. REGISTER FOR TESTS: Complete and submit this online form after talking to your teacher about which exam to sign up for.

2. MAKE A PAYMENT: Pay the registration and exam fees through the NPHS Online Student Store.

The cost of taking IB exams is $125 per exam. Credit card payments are accepted online through the NPHS student store at https://newburypark.revtrak.net/IB-Tests/#/list .

If you are in need of financial assistance AND have been approved for the free and reduced lunch program for the 2020-21 school year, contact Ms. Dogancay via email by October 15 for more information.

The final deadline for IB registration and payment is FRIDAY, OCTOBER 30, 2020. Unfortunately, IB testing fees are non-refundable and therefore refunds cannot be issued after this date.

After this form has been submitted, any changes to your IB registration must be approved via email by Ms. Dogancay on or before October 30. A copy of your responses will be sent to the email address that you list as your "Student Email" below, upon completion of the form.
