Mt Colah Football Club - 2017 Season
Application for Registration Fee Payment Plan
This application form is for players wishing to enter into a Fee Payment Contract with Mt Colah Football Club for their 2017 Registration Fees.

- Eligibility for a Fee Payment Contract is assessed and approved by the Treasurer and/or the Club Executive Members.
- All players wishing to apply for a Fee Payment Contract must provide valid and reasonable grounds in this application.
- A Fee Payment Contract is limited to the Player's Registration Fees for the 2016 season only.
- Completion of this application form on its own is not approval for a Fee Payment Contract .
- Mt Colah Football Club reserves the right to reject this application.
- All approved payment plans must by fully paid by 30 June 2017

Once this application has been submitted the Treasurer will consider the application and advise if a Fee Payment Plan is to be approved.
