Thank you for contacting Harleigh Cemetery Association. This is the work order form for the following cemeteries:

Please fill out all required information in order for us to accurately locate your relative.

Company Policy on Work Orders:

Only the next of kin/plot owner can request work orders. Orders by others will not be processed.
Work orders can take 8 to 12 weeks to complete.
There is a $100 fee for headstone resetting of burials more than 10 years old.
