Summative Reassessment Reflection
The goal of summative assessments is to demonstrate student mastery of concepts and skills.  You are welcome to retake summative assessments to replace your previous score. You will then work with Mr. Polutnik to schedule your retake based on his availability. 

REASSESSMENT GUIDELINES

  • If a student wishes to retake a summative assessment, this reassessment reflection must be submitted to Mr. Polutnik within 5 school days of the original grade being posted. 

  • Mr. Polutnik requires that students complete and pass 75% of formative assessments before a student retakes a summative assessment.  

  • For reassessments, a student’s higher score will replace a lower one. 

  • Students are given at least one opportunity to reassess for each summative assessment. Subsequent reassessments are subject to Mr. Polutnik's discretion. 

  • Summative assessment retakes must take place within 10 school days of the initial grade being posted.

  • Reassessments must take place prior to the date posted on the Assignment Submission Deadline table (see table in the DOHS Schoolwide Expectations).

  • Reassessment accommodations in IEPs and/or Section 504 Plans supercede District reassessment guidelines.
