Room Reservation Request Form

EFFECTIVE IMMEDIATELY, Room Reservation Requests will be accepted from faculty, staff and administration ONLY. Students wishing to reserve a room for Howard University-related musical activities should ask a member of the faculty to sponsor their use and to submit a request on the student's or student group's behalf.

***Submission of this form constitutes a request to use a room, and does not necessarily guarantee availability or that permission will be granted. PLEASE DO NOT ASSUME YOUR REQUEST IS AUTOMATICALLY APPROVED WITHOUT RECEIVING WRITTEN EMAIL CONFIRMATION FROM THE ADMINISTRATOR.

Prior to submitting your request, please check the online room schedules for possible conflicts with previously scheduled events.

To Faculty Sponsors of Student Groups:

By submitting this form you are agreeing to be present at the event, including picking up keys, overseeing any loading in and out of equipment before and after the event, and being responsible for making sure the room is neat and secured at the end of the event.

Keys will be given only to you and must be returned by you to the Department of Music Office the next business day unless other arrangements have been made. Under no circumstances are students to be given keys or made responsible for securing the room after the event.
