Part 4
Free school meals Application form (Nursery – Year 6)
Every pupil in Reception, Year 1 and 2 will be eligible for free school meals. However we ask that ALL PARENTS and CARERS fill in the attached school meals registration form, including parents with children in Reception, Years 1 & 2. This is important so that we can continue to register pupils who attract the pupil premium grant, which is worth £1,300 a year per pupil for our school.

Part 6: School trips and other off-site activities (All Pupils)
The trips and activities covered by this consent include; all visits (including residential trips) which take place during the holidays or a weekend adventure activities at any time off-site sporting fixtures outside the school day, all off-site activities for nursery schools. The school will send you information about each trip or activity before it takes place. You can, if you wish, tell the school that you do not want your child to take part in any particular school trip or activity. Written parental consent will not be requested from you for the majority of off-site activities offered by the school – for example, year-group visits to local amenities – as such activities are part of the school’s curriculum and usually take place during the normal school day.
Part 7: Swimming Lessons (Year 6 only)
Year 6 pupils will be attending Church Farm swimming pool on a weekly basis for the duration of this year. It will start on the 16th September.Your child must bring their swimming kit to school every Friday. Swimming kit includes a towel, a one-piece swimming costume for girls or a pair of swimming trunks for boys (long shorts are not appropriate), and shower gel. Girls are to bring their own swimming caps as the swimming pool will not be providing them. Your child can wear a pair of swimming goggles if you give your consent below.Children are advised to leave earrings (except for a pair of studs) and any valuable items at home. Perfumes and aftershaves are not permitted.
