Kindergarten Registration and Screenings
Thank you for your interest in joining the Mustang Family!  Please fill out this form to begin the Kindergarten registration process and book an appointment to register your child on the following days; Tuesday, May 24th, Wednesday,  May 25th and Thursday, May 26th. Your child will need to be present at the time of registration.  We will complete Kindergarten screenings during the registration process.  All parents need to complete Final Forms located at http://westfall-oh.finalforms.com. If your student is new to the district, select "New Account" when registering your student(s).  If your child is currently enrolled in Westfall, you are required to sign-in and update their student records in Final Forms.  *Please complete this process prior to your scheduled appointment.

When you arrive at your appointment time please have the following documents:
Parent/Guardian Photo ID
Your child's birth certificate
Your child's social security card
Your child's shot record
2 proofs of residency
Court documents (if applicable)

Please keep in mind that if you are missing any required documents, we will not be able to register your child.
