Use this form if you need to request a login for the Lumen Parent Portal System. This system will allow you to access to your students' grades, lunch account balance, health room visits and contact information. You only need to complete this form once, even if you have multiple students in multiple buildings. Please note: Only custodial parents/guardians who are considered "Educational Decision Markers" can be given Parent Portal Accounts.

Parent Portal login information is sent via email from tech@stcmo.org. Please list a working email address that you use often.

Please list the names of the students you need accounts for *
You will receive login credentials for each student that you list. You can combine those accounts to just one login, instructions are in the Parent Portal Instructions on the school website, under Parents tab. Please list your students' first and last name and separate each student's name by a comma.
