Schedule Conflicts: Training for these teams will take place on Sundays and/or Wednesdays at Total Turf in Pitman, NJ. Times TBD. Please list any regular conflicts you have on these days that may prohibit your ability to participate on a particular team (for example: "LAX practice on Sundays from 2-3pm" OR "on vacation 7/18-7/22") *

After submitting this form, please mail the following to 1003 Tanglewood Drive, Erial, NJ 08081 in order to secure your spot at evaluations:

1. A completed waiver (https://southjerseyedge.files.wordpress.com/2018/02/sj-edge-waiver-2017-1.pdf)
2. For new players only, a $50 registration fee. Once selected for a team, you will be expected to submit a $200 deposit immediately.
3. For returning players, a $200 deposit. This deposit MUST be paid prior to team placements.

The first placement dates will be as follows:
- Wednesday, March 13th: U12 and U14 ONLY, sometime between 5:45-9:00pm. Times to be emailed once registration is complete.
- Sunday, March 17th: All age groups, sometime between 6:00-8:00pm. Times to be emailed once registration is complete.
- Wednesday, March 20th: All age groups, sometime between 5:45-9:00pm. Times to be emailed once registration is complete.

Other dates will be added as needed to complete team placements.
