camdisc membership form
Membership is free and open to anyone, with parental consent being mandatory for anyone under the age of 18. 
This membership list will last at least until the 2025 club AGM. You may terminate your membership at any time by emailing camdisc.org@gmail.com.

Please use a personal email address (i.e. not @cam.ac.uk) associated with a Google account where you can receive emails, to allow full access to the mailing list and organisational pages.

If this form is filled out on behalf of a person under the age of 18, it must be completed using the guardian's email address as we cannot store contact details for under 18s.

The club welcomes members under the age of 18 as per our Safeguarding Policy, but a separate parental consent section will need to be filled out by a parent / legal guardian.

In gender-restricted play, camdisc endorses UK Ultimate's eligibility policy of an "inclusive and equal approach welcoming all players to play in the division (or gender role in the mixed division) that they identify with.".

Most of camdisc's events do not have gender restrictions.
