Evaluation of chapter activities is based upon the criteria of education, implementation, outcomes and creativity and innovation. Please refer to the Community Health Challenge grading rubric for specifics on the criteria listed. Supplemental materials such as pictures, flyers, presentation slide decks and etc. may be emailed to email@example.com. You are encouraged to upload photos to the NCPA Student Network group page on Facebook. These materials will only be used for grading purposes in the chance of a tie. Additionally, these materials may be used to highlight innovative and progressive projects in America's Pharmacist, the NCPA student website, or in the NCPA monthly student e-Newsletter. Photos submitted in .jpg format are encouraged.
Final reports should be sent electronically to firstname.lastname@example.org by June 5, 2018. Please be as accurate as possible and answer all questions with honesty and integrity. Challenge Reports must be email time stamped by 11:59 PM Eastern on June 5, 2018. Only one application will be accepted per chapter. The winner will receive $250 for the NCPA student chapter, which will be presented at the NCPA Foundation Awards Ceremony at the NCPA Annual Convention.
