ACCESS CHRC Student Internship Application
ACCESS Community Health and Research Center student internships are unpaid internship practicum experiences coordinated with a school program. For students interested in completing an internship at the ACCESS Community Health and Research Center, please submit this form by the indicated deadline below depending on your anticipated start date. Fall (September - December): July 15th
Winter (January - April): November 15th
Summer (May - August): March 15th
*For students completing a mental health internship, the preferred application deadline is 6 months before desired start date.

Are you a current student?
Eligible students must currently be enrolled in an academic institution.
Degree Program and Concentration
Please indicate both the degree sought out (ex. Master of Public Health, Bachelor of Arts) as well as your major/concentration in that degree program (ex. Health Behavior Health Education).
Grade Point Average (GPA)
We require a 3.0 minimum GPA for consideration.
Current Employment Status
Do you speak any other languages besides English?

Our internship program works in three seasonal cycles: Fall, Winter, and Summer. While exact dates are subject to discussion and mutual agreement, please select the cycle that best fits your availability.

For students completing a mental health/counseling internship only, please indicate what credentials (if any) you require from a supervisor in order to complete your internship.

At ACCESS, we require a minimum of 8 hours per week for student interns. Please select the desired number of hours that you would be willing to commit. You may select multiple options to indicate flexibility.
