Fraternity House Supplies Request Form

Fill out this form if you are running low on one of the listed supplies. Supplies are provided out of a set allotment. Additional needs for supplies, (such as toilet paper) will need to be purchased by members of the house.

If you have a question about an item or how much of it your house is supposed to have per year, see the screenshot below or contact Facilities Operations. For supplies not listed, please utilize the additional monthly cleaning/maintenance stipend distributed to House Corps.

Please note - complete this form in advance of your immediate cleaning needs. For example - do not submit Friday at 11:30 am if you need more supplies for group cleaning Friday afternoon.
