Course Reserves Form for Instructors
After completing the online form, please stop by the library to drop off the physical copy. A short bookmark form must be completed and handed in with the material. It may be found at the circulation desk.

You will receive an email notification if one of your course reserves is overdue. Please note that the Library is not responsible for lost or damaged course reserves.

NOTE: Due to the library's system migration, all record creation is undergoing a freeze. New course reserves cannot be processed until the second week of February. Form submissions will still be accepted, but please be aware of the time delay.
