Salmon River Central School District's top priority is the health and safety of all students, staff and communities in our district. Given the extraordinary circumstances associated with the COVID-19 outbreak, students will only be picked up and dropped off from one location for school bus transportation.

We ask that parents/guardians fill out one request form per student.

**This form MUST be filled out to receive transportation for your student.
