Please check yes or no for each meet that your swimmer/diver plans to attend so our coaches can plan in advance. YOU WILL NEED TO COMPLETE A SEPARATE FORM FOR EACH SWIMMER IN YOUR FAMILY. There will be an option for another entry after submission. Coaches must have your availability by Sunday at 5PM the week of the respective meet. We understand there may be changes to what you are submitting now, and that is ok to update so long as done so by Sunday at 5PM before the meet.
