Kennedy-King College Library Course Reserve Request
The Library can place materials (books, DVDs, maps, archives, etc.) on reserve for your students. Please fill out this form, submit, and then bring your items to the Library's Reference Desk for your items to be on reserve (Let the Librarian know that you filled out your form online). You must fill out one form per item. A Librarian will contact you once your item has been shelved. Please contact Alecia Kerr if you have any questions.
CD-ROM (saved documents or music)
Artifacts (sculptors, art work, newspaper clippings)

Course name and number/ Instructor's name
Note: Use Dept. name or Dept. Chair name if more than one instructor is teaching the same course. Also, write down all the course numbers for each class the item is for.

Never submit passwords through Google Forms.
