I acknowledge that I will be attending this Brazilian Jiu Jitsu program and with any Martial Art, it encompasses physical, mental and social agility and participation. Our mission statement continues to provide ample spaces for children and adults to partake in the art of Brazilian Jiu Jitsu in an environment that allows everyone to learn, be safe and have fun. With that being said, if there are medical (of any form) concerns, they must be presented in order to accommodate to your (or child(ren)'s) learning skill. I agree to present any medical (of any form) concerns prior to your (or child(ren)'s) start date. *

Photograph & Video Release Form

I hereby grant permission to the rights of the image use of my (child(ren)), likeness and sound of my/their voice as recorded on audio or video tape without payment or any other consideration. I understand that my/their image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my/their likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my/their image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. Photographic, audio or video recordings may be used for the following purposes:

• conference presentations

• educational presentations or courses

• informational presentations

• on-line educational courses

• educational videos

By signing this release, I understand this permission signifies that photographic or video recordings of student's may be electronically displayed via the Internet or in the public educational setting. I will be consulted about the use of the photographs or video recording for any purpose other than those listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only.

By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.

Within the contract lies the agreement between the students of the program and the instructor(s), Agreement precedes as follows:

1. Attire/Hygiene

No Gi: Shorts, spats/rash guard (if needed), sweats, shirt (long/short sleeves spandex if possible i.e under armor) Young Ladies: sports bra and the above attire

Gi: Kimono/Gi, shirt (long/short sleeves spandex if possible i.e under armor), spats (if needed) Young Ladies: sports bra and the above attire

Improper Attire: Jewelry, tight clothing, socks, jeans

Hygiene: Prior to starting class students must practice proper personal hygiene for their own safety and that of their peers. Which includes but not limited to: 

·       Nails clipped and/or filed

·       Gi/ No Gi attire washed, deodorized and free of defects (i.e rips and/or tears)

·       Hair tied/Braided if needed

2. Conduct/Behavior

Jiu Jitsu and Judo for All Inc., is committed to the health, safety, and overall wellbeing of its students and staff on and off the mat. Therefore, Jiu Jitsu and Judo for All Inc., will not tolerate unreasonable, inappropriate, threatening, obscene, crud, harassing, illegal or indecent behavior of any kind. Jiu Jitsu and Judo for All Inc., along with the staff hold the right to dismiss any student from the program at anytime if any of the above contractual behaviors are breached.

 3Waiver, Release and Assumption of Risk

In consideration of being permitted to participate in any way in the Jiu Jitsu and Judo Program indicated below the participant and/or legal guardian(s) of the minor participant named below agree:

A. The participant and/or legal guardian(s) will instruct the student that prior to participating in the above physical educational activities or event, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate.  I understand and agree that, if at any time, I feel anything to be UNSAFE; I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further.                  

B. I/We fully understand and acknowledge that:

i. The undersigned participant and/or parent guardian understands and agrees the student is voluntarily participating in physical educational activities, i.e Brazilian Jiu Jitsu and Judo, which may expose student to some level of risk, injury, and and/or total disability, paralysis and death. The student after participating in several weeks of an introductory course is fully aware of the nature of the activities and agrees to accept any and all risks associated with participation in these activities.

ii. Participant and or parent/guardian acknowledges that student is in good physical and mental health and that student and or parent/guardian will notify both Jiu Jitsu and Judo for All Inc., and institution of any changes via written notice from either a licensed medical professional or that of the parent/guardian.

iii. The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe.

iv. These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below.

v. There may be other risks not known to us or are not reasonably foreseeable at this time.

 C. I/WE accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death, however caused and whether caused in whole or in part by the negligence of the Releasees named below.

D. I/We HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the martial arts facility used by the participant, including it owners, managers, promoters, lessees of premises used to conduct the martial arts event or program, premises and event inspectors, underwriters, consultants and others who give recommendations, directions or instructions to engage in risk evaluation or loss control activities regarding the martial arts facility or events held at such facility and each of them, their directors, officers, agents, employees, all for the purpose herein referred to as “Releasee”…From all liability to the undersigned, my/our personal representatives, assigns, executors, heirs and next to kin For any and all claims, demands, losses or damages and any claims or demands therefore on account of any injury, including but not limited to the death of the participant or damage to property, arising out of or relating to the events(s) caused alleged to be caused in whole or in part by the negligence of the releasee or otherwise.

E. I/We HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage.  Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.

 F. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.

G. On behalf of the participant and individually, the undersigned partners(s) and/or legal guardian(s) for the minor participant executes this Waiver and Release.  If, despite the release, the participant makes a claim against any of the Releasees, the parents(s) and/or legal guardian(s) will reimburse the Releasee for any money which they have paid to the participant, or on his behalf, and hold them harmless. 

I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WTHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
