The purpose of this form is to be used by active alumni chapters to request prospective student's and/or current student's contact information, in order to engage in recruitment and retention efforts.
Please note: Requests must be submitted 30 days prior to event, to allow adequate processing time. Please allow 10 business days for response.
Responsibility of Usage:
* Should be used for recruitment and retention purposes only.
* Should not be used for personal reasons.
* Should be used for the intended purpose of the initial request.
* Should be submitted for each program to ensure most current student data.
