If you're an incoming freshman or new student and the school year hasn't started yet (so you don't have an @
mysohi.org email address yet), you may use a different email address.
Everyone else MUST use their Seeds of Health email address (@mysohi.org).
A parent/guardian will also have to fill out the parent online forms below before you can participate. The parent forms are different than the student forms. The link to the parent forms will be provided at the end of this form.
