This form is for members to request their meals be placed in a meal box (1) for pickup in the Servery during service hours or (2) for later pickup in the refrigerator located in the basement of Graham House. Members may alternatively elect to substitute one meal box request per month for a meal voucher, which can be redeemed for a meal at a later time of their choosing.
