This is an application form for the 12-week Fit For Life exercise programme delivered by Momentum in Fitness Charity, starting on Wednesday 7th January 2025

Please note that this application does not guarantee a place on the programme. We will review all applications and then shortlist up to 12 participants who will be invited to sign up no less than 2 weeks before the programme start date.
We collect participant demographic data, periodic survey data and feedback for the purposes of evidencing project impact(s) and for grant/funding applications (data used for these purposes will be anonymised) .
