Marching Band 2017-18 Info Form
Please fill out completely for each student participating. There is no need to print this form - just "submit" at the bottom. Please go to www.registermyathlete.com to submit the physical and medical paperwork. Physicals can be completed between March 1 through May 1. All paperwork is due to no later May 1 in order for student to attend Marching Band Summer Camp.

The $50 Tempe Union Activities payment can be made at book distribution in Late July (after Camp).

Band Camp runs from July 17-22 at Pine Summit Camp in Prescott. If you pay by April 28 the fee is $400 (our "Early Bird" rate). If you pay between May 1 - May 26, the regular fee is $475. Please contact the Band Office if you need to set up a payment plan - aholste@tempeunion.org. Band Camp fees can be paid with tax credit, or paying directly to the school bookstore (checks made payable to "Corona del Sol" with student's name and "band camp" in the memo line).

Parents: I want to sign up for the Band Boosters
Band Booster Membership is $30 for the year and includes a Marching Band season DVD/Blu-ray, Basha's reloadable gift card, membership directory, and more! Support the band! We encourage all families to become a member of our booster organization (if you have more that one student in the band - the $30 covers the entire family). Checks can be made payable to: CDS Band Boosters (OR you can pay on our online Square account). Make sure to write "Booster Membership" in the memo line and mail checks to: Corona del Sol Band, 1001 E Knox Road, Tempe AZ 85284.
