Special Event Request Form For Diapers
REQUEST INFORMATION: Thank you so much for reaching out to Miami Diaper Bank about receiving diapers for your special event. We allocate a certain number of diapers every quarter to support special events in the community which are designed to serve low-income children and families in South Florida with diaper need. We currently do not provide on-going support with diapers to organizations who are not an official Miami Diaper Bank core community partner agency. However, If you complete this form to request diapers for your special event, please note it is a one-time request and we will only be able to provide diapers for the event if we have them available AND your organization meets all of the requirements outlined below. 

DEADLINE: Please be advised that we must receive a completed Special Event Request Form from your Organization a minimum of 45 business days prior to the event date to receive consideration. This allows our team enough time to evaluate the request and identify the diapers we have available (if any) for your special event.  

REQUIREMENTS: Organizations requesting diapers from Miami Diaper Bank for their Special Event must meet the following requirements:
-Event must NOT be open to the general public
-Organization must provide tent/table/chairs as needed for Miami Diaper Bank at the event
-Organization must ensure that all families in attendance who are receiving diapers are at an income level which falls below the Federal Poverty Line and/or they are receiving other government assistance at that time which would qualify them to receive diapers from Miami Diaper Bank 
-Organization must provide 3-5 volunteers to help Miami Diaper Bank with diaper distributions at the event
-Organization must provide a post event report of the total children and families served as well as send Miami Diaper Bank photos and/or written/verbal testimonials of the impact of the families receiving diapers
-Organization must pick up the diaper order at Miami Diaper Bank's Diaper Hub and transport the diapers to the event unless otherwise agreed to by Miami Diaper Bank staff
-The request must be to serve less than 15 babies at the event (Please note we provide up to 50 diapers per child being served). If you need diapers for more than 15 babies we may be able to provide you with additional diapers for a discounted fee or sponsorship. 

REQUEST DETERMINATION: Please note that our ability to support your special event will be on a first come, first served basis. If you fill out and submit this Special Event Request Form, we cannot guarantee that you will receive diapers as it is based on our available inventory for Special Events. Once we have reviewed your Special Event Request, we will reach out to your organization to let you know our decision. To review and evaluate each request, organizations selected will be notified with no less than 2 weeks prior to the event date and if any additional information is needed, we will contact you directly.
