FALLSTON RECREATION COUNCIL (FRC) INCIDENT INFORMATION
Please use this form to submit information regarding an incident that recently occurred at a FRC practice, event, game or meeting. Information entered into this form will be sent to the FRC Board or Directors and Program chairs to provide board awareness, outline disciplinary follow up or actions taken, as well as maintain a log of incidents that occur across all programs. All parties are asked to keep this information confidential in nature to the greatest extent possible as used only in upholding the FRC code of conduct, which can be found on our website at
