This form is for my program's record keeping, and so I have access to emergency information. There are moments that this is a high-performance activity. This is how I gather and set up my emailing list to give updates. This is also what I use to do bulk orders for shirts, shoes, and gloves. This is how I gather allergies so I can pass on allergen info anonymously to our Boosters when they purchase/prepare food for the group. Everyone joining marching band needs to fill this out.
The fees for marching band are a $300 Booster Fee. This covers trip planning, show purchases for music and drill, staffing, etc. There is a separate $290 ($50 for Summer and $240 for Fall) activity fee paid through the Parent Vue/Student Vue portal. The $290 covers the activity fees regarding summer and fall events/rehearsals as well as reserving facility spaces such as the three practice fields and school facilities. In the case of emergency, I require at least 1 parent phone number, name, and email. There will need to be an up-to-date sports physical on file before you have access to registering. You will not be able to march without an up-to-date sports physical on file.
