Mt Hebron HS Transcript Request/Waiver Form for 9th-11th Grades and Dual Enrollment

Please complete one form for each transcript request. THIS FORM IS NOT INTENDED FOR SENIORS.  Seniors, please contact your Counselor for the proper transcript request procedures for the Class of 2021.

By completing this form, you are verifying that this has been completed with your parent/guardian(s) approval. Please note that when you complete and submit the Transcript Request google form you are giving HCPSS permission to send the transcript through email. In addition, you are giving HCPSS staff the permission to complete evaluations associated with your child’s college application and/or scholarship applications.

Release of Student Records
The law requires that schools receive written permission signed by the parent/guardian before transcripts and other student records can be released to a third party.  In lieu of a parent's signature, completion of this form verifies it has been completed with the approval of your parent/guardian.

Do you need a Counselor Letter of Recommendation? *
Colleges and universities prefer that counselor and teacher evaluations and recommendations be confidential. Colleges generally believe that recommendations written with this understanding are more candid and honest. Therefore, those recommendations have more validity and carry more weight in the admission process than recommendations that parents and students can access. In light of this, we request that all students/parents requesting letters of recommendation answer the following waiver question. If you choose not to waive your right to access letters of recommendation, this information will be included in the counselor and teacher’s letter. *
