Whether you chose to sell off your inventory,  returned it to LuLaRoe, received your refund check or are still waiting for your check we want to hear from you.  Please fill out this survey and share it with any other former LuLaRoe consultants that you may know.  There is a power in numbers and we need to ban together.  We want to connect with as many as we can.

The information collected will NOT be sold and will remain confidential and will only be used if needed and if the information is not needed by the FTC, FBI and Attorney General then it will be deleted.  This survey & petition is being collected by me (Kimberly Flynn) former LuLaRoe Consultant joined in Nov. 2015 resigned in Oct 2017.  All the info is being stored on a separate google drive, with which I only have access to.  I am a person of integrity, a single mother and a crusader to this cause.  I was also mentioned in the Bloomberg Newsweek Article.  

If you do not feel comfortable giving your address then by all means put N/A.  The reason for collecting the information was to show valid names & addresses for credibility versus just names and email address.  It's more of a backup in sense.  Also in case any of these organizations need to contact to you to verify the facts.
