Carroll Choir Disney Trip Chaperone Questionnaire
In an effort to provide a safe and enjoyable experience for our students we screen and select Chaperones to assist with travel. Our trips have limited space and in an effort to provide as many students the opportunity to travel as possible we must limit adult participation in trips. At the completion of student enrollment we will determine the needed number of Chaperones and notify selected Chaperones via email to collect deposits and communicate a final price for the trip.

Chaperones are required complete a Carroll ISD Volunteer Application and be cleared by the district in order to travel with the choir.
Chaperones will be assigned to small groups (7-10) of students throughout the trip for room checks, wake up calls, and some small group activities during travel.
Chaperones will be notified via email when chaperone decisions are made. Payments should begin once a chaperone has been notified.
Chaperones must participate in all group activities during the trip and cannot be independent from the group during the group activities.
A copy of your responses will be emailed to the address you provided.
