Application for Sr. High Camp
Complete this form for your students. Please mail checks to AGQBA to be received at least two weeks prior to camp and make sure it is to cover each of your students and coaches who have been confirmed. You will receive an invoice by email after Nov. 7. Make checks payable to AGQBA. Cost is $100 per student and $50 per coach (reduced cost for coaches as a thank you for letting us put you to work during camp!).

Please note: You will be responsible for paying for all students you register for camp. You may transfer that registration to another student, but if another student can't be found to replace the original registrant, your school will still need to pay for that slot. Please let me know if you have a student that isn't able to attend, as we may be able to offer that slot to students on our waiting list from other schools, which would transfer the burden of payment to the new school if they accept the slot.

Each coach is able to register 4 guaranteed participants and 2 waitlisted participants. After the registration deadline, we will let coaches know if we have additional spots for their waitlisted participants. If all spots are filled and we still have room, we will reach out to coaches again for additional participants.

Minimum registrant threshold: To hold camp, we need at least 85 participants registered by the registration deadline. If a camp doesn't meet that threshold, it will be cancelled and registered participants will be offered the opportunity to attend another camp. If they are not able to attend, a refund will be issued if payment has already been received.

*New this year: T-shirts are available separately for pre-order. We may have a SMALL and LIMITED number available for purchase at camp.

Camp Dates: Nov. 21 @ 1pm - 22 @ 4 pm, 2025 (This IS the weekend before Thanksgiving Break)

*Each student attending will need a Parent Authorization form, which includes a Medical Authorization form, and a Camper Code of Conduct form. Please print these and have each student/guardian sign them.*
