This form initiates the process to become a Level I, II, or III volunteer and will serve as both the Volunteer Release/Criminal History Release Forms for Atlanta Neighborhood Charter School. The background check will be conducted by our third-party screening partner, DISA. You will receive a link to the email you provided in this form to complete your background check. Please provide all requested information for fields and make sure to double-check your data entry to ensure correct spelling and correct numerical sequences.

Once your background check has cleared, your name will automatically be added to our Approved Volunteer List for the school year. Teachers and Faculty will reference this list to confirm eligibility for volunteering.

Please understand that Atlanta Neighborhood Charter School reserves the right to refuse the services offered by any volunteer. Please let us know if you have any questions or concerns.
