SDM Summit 2024: Sponsorship Form
Disability Rights Nebraska is hosting a summit on supported decision making, guardianship, and self-determination on Saturday August 24, 2024 at the Barbara Weitz Community Engagement Center  at the University of Nebraska Omaha. The event will run from 8:30 AM (registration/coffee and social time) 
until 3:00 PM. 

To help defray costs, we are seeking sponsors for financial contributions.  

Contributions are tiered as such:

Tier 1 Sponsor: $500 and above
  • Benefit: Award, recognition at event, vendor fee waived 
Tier 2 Sponsor: $250 and above
  • Benefit: Award, recognition at event, vendor fee waived 
Tier 3 Sponsor: $100 and above
  • Benefit: Recognition at event 
Tier 4 Sponsor: $25 and above
  • Benefit: Recognition at event
Deadline to submit Sponsorship Form is August 21st at 5:00 PM (CST). Here is the link to submit payment.
