COVID-19 in CDCR: Survey for people outside to share what they are hearing from people inside
You Can Download and Mail the Version of This Form For People Inside Here: https://www.prisoneradvocacynetwork.org/covid-19.html

These questions are designed to inform advocacy efforts around the spread of COVID-19 in prison. We are hoping to learn what harm it is causing and how to decarcerate based on this harm, and what efforts may be most successful. The more specific you can be with descriptions, dates, times, locations, numbers, and people in your declaration, the better. If you have any documents that relate to or support an answer. This survey will be used to gather examples to use in legal demand letters, audits, legislative hearings, litigation, media work, and other processes. PAN is a volunteer organization that does not have the capacity to accept intakes. PAN will not be conducting advocacy for any individual as a result of this survey. This survey does not create an attorney client relationship with PAN. This survey's purpose is to gather evidence about the consequences of COVID-19 in prison and support broad organizing, pro-se litigation, and education efforts, not individual case work.

Only the first section is mandatory, but please share answers to as many other questions as you feel comfortable. The more specific you can be with descriptions, dates, times, locations, numbers, and people the better. The questions are long because they mirror the information you would need to present to CDCR, the Governor, or Courts in asking for release and are designed to educate the survey-taker about what those decision makers deem important for them to know right now.
