Summer Camp 2023 Registration Form
Join us for the 2023 Musical Theatre Summer Camp. This camp is for kids and young adults entering grades 1-12. The one camp session includes a camp T-Shirt. Space is limited and reserved on a first-come, first-served basis. There is no prorating, and no refunds after May 20, 2023. Payment is due on DUE DATE. Please be aware there will be a 1.5% interest charge per month on late invoices. Campers/Volunteers must bring their own water bottle, lunch, snacks, an extra face mask*, and a personal hand sanitizer. Each participants is responsible for purchasing their own costume.

Having a regular creative outlet is not just part of a well rounded education – it is essential for a child’s emotional health and well being.  Producing a show brings the arts together and provides growth opportunities for all varieties of learners.  For example, kinesthetic learners engage their bodies through dance, linguistic learners dive into the written and spoken word, spatial learners tackle set and costume design, and musical learners revel in the show’s score.

Besides being incredibly fun, musical theatre helps young people develop many of the skills necessary for success in today’s world:  
+Self-Confidence:  Throughout the rehearsal and performance process, young performers discover how to take creative and emotional risks and learn to trust their abilities.
+Literacy:  Students increase reading and writing skills at their own pace by immersing themselves in the story, reading and re-reading their scripts, memorizing their lines and taking notes at rehearsal.
+Communication:  During rehearsals, students learn how to articulate their thoughts and questions and carefully listen to direction in order to improve their performances.
+Imagination:  Theatre is most dynamic when the imaginations of its participants are fully engaged. Students make countless interpretive choices in design and performance to create a detailed, fictional world for their audiences.
+Empathy:  Through character work and story creation, young performers explore the lives of others. First-hand perspectives of new circumstances challenge prejudices and aid in the development of well-rounded students and citizens.
+Critical Thinking & Problem Solving:  The amount of work required to successfully put on a show makes all participants valuable troubleshooters. Whether inventing mnemonics to remember lines, adjusting staging to show off everyone’s faces, or mastering a tricky scene change, students have countless opportunities to creatively tackle challenges.
+Self-Discipline:  Through prompt attendance at rehearsals and learning lines, music and choreography on schedule, students acquire valuable self-discipline techniques and become reliable and trustworthy members of an ensemble.
+Community Awareness:  A theatrical production provides ample opportunities to connect with our families and neighbors.  Students can engage parents, community centers and local vendors to help create sets, props, costumes and publicity.
+Public Speaking:  Whether performing for their peers during rehearsal or for their school and local community during a performance, students gain expertise and confidence speaking in front of a group.
+Curricular Integration:  A musical provides a rich and engaging springboard for students to explore many subjects in their classrooms and discover connections among various modes of learning.  Our ShowKit came with show-related lesson plans for English Language Arts, Math, Social Studies and more – all of which fulfill the Common Core State Standards.
+Teamwork:  Theatre requires extensive creative input from all its participants. Students build ensemble by working together to rehearse, construct, promote and perform their show.  Every role – whether onstage or behind the scenes – is vital for the show’s success.

As you can see, our campers will gain valuable life skills through their participation in camp.  We’re looking forward to jumping into camp activities and rehearsals.

Consent
I hereby freely and voluntarily consent to participant’s participation in the summer camp described above. I agree to assume all financial responsibility for participation in the activities and hold Fiol Studio, LLC and all of their corporate members, affiliated entities, employees, officers, directors, and agents (hereby referred to as "Sponsors") harmless for all costs incident to participation in this summer camp.

I, the undersigned, on behalf of the participant in the summer camp described above, do waive and release Sponsors from liability for any injury, accident, or damages caused by any vehicle, weather, sickness, or otherwise stemming from any act or omission of any individual. I also release Sponsors and agree to indemnify them with regard to any financial obligations incurred by participant’s acts or omissions.

I understand that summer camp activities involve some risks, and I hereby agree to assume and consent to such risks. I hereby waive and release Sponsors for any injuries, damages, or losses incurred in connection with actions, omissions, conditions, or developments within or outside Sponsors’ control. By my execution of this document, I voluntarily assume all risks involved in such activities, whether expected or unexpected. I hereby acknowledge that I have been warned of such risks, and that I have been advised to take appropriate action and to govern myself accordingly.

I hereby grant Sponsors full authority to take whatever actions they may consider in their sole discretion to be warranted under the circumstances concerning participant’s health and safety and I specifically and fully release each of them from any liability for such decisions or actions as may be taken in connection therewith. I authorize Sponsors at their discretion to place participant, at my own expense and without further consent, in a hospital that is readily available, and to place participant in the hands of a local physician for treatment should the need arise at my expense.

I agree to comply fully with the rules of Sponsors and I agree that Sponsors have the right to enforce their standards of conduct as determined and interpreted in their sole discretion, and that, should participant fail to comply with them, Sponsors have the right to terminate participant’s participation in the program. In addition, I acknowledge the right of Sponsors to terminate participant’s participation at any time for failure to maintain standards or for any actions or conduct for which Sponsors deem incompatible with the interest, harmony, comfort, and welfare of other participants.

All references in this release to Sponsors shall also include all of their administrators, employees, volunteers, and agents. All references to the “parents" of the participant include the legal guardians or other adults responsible for the participant.

I hereby grant to Sponsors the right to photograph and/or videotape participant and further to use participant’s name, face, likeness, voice, and appearance in connection with exhibitions, publicity, advertising, and promotional materials without any reservation, limitation, or consideration. This waiver specifically releases any common law causes of action or claims under Fla. Stat. 540.08 and expressly constitutes written consent for publication of my name, face, likeness, voice and appearance.

I have read the terms and conditions set forth by Sponsors and I agree that this constitutes a part of any agreement with Sponsors. I understand and agree to all of Sponsors’ terms as set forth in their descriptive information and in this Release.

I certify that I am the parent or legal guardian of the above-named participant, and that I have read the foregoing release and examined the information in the description. I hereby join in each and every part of this Consent and Release (including such part as may subject me to personal financial responsibility) and hereby relinquish any claims that I may have against Sponsors as set forth above, both in my own behalf and in my capacity as legal representative (as applicable) of the participant, including without limitations any claims arising as a result of the participants leaving the supervision of Sponsors. I agree that if any portion of this document is found to be void or unenforceable, the remaining portions shall remain in full force and effect.
