Thank you for allowing your student to attend the Attic After School. We are a Christian non-profit that seeks to help students 7-12th grade to write a better story. Filling out this form will allow us to care for your student and ultimately see their lives changed. 

Students need to have their parent or guardian fill out this form in order to attend the Attic After School or to receive a ride home for the current school year (2023-24).
