The Whitko Reporting Form should be used to report alleged incidents of peer conflict, bullying, harassment, or intimidation that occurred during the current school year on school property, at a school-sponsored activity or event off school property, on a school bus, on the way to and/or from school, or through personal mobile device on school property. School staff will address incidents that occur at school or have a connection or nexus back to the school setting that create a risk of harm to other students while they are at school or interfere with the educational environment. If you are a student, the parent/guardian or a family member of a student, a school staff member, or a bystander and wish to report an incident of alleged peer conflict, bullying, harassment, or intimidation, please complete this form. BULLYING- By Indiana statute IC 20-33-8-.2, means overt, unwanted, repeated acts or
gestures, including verbal or written communications or images transmitted in any manner
(including digitally or electronically), physical acts committed, aggression, or any other behaviors,
that are committed by a student or group of students against another student with the intent to
harass, ridicule, humiliate, intimidate, or harm the other targeted student and create for the targeted
student an objectively hostile school environment.
HARASSMENT-By Indiana statute IC 35-45-2-2, person who, with intent to harass, annoy, or alarm another person but with no intent of legitimate communication: makes a telephone call, (whether or not a conversation ensues) communicates with a person by telegraph, mail, or other form of written communication to: communicate with a person; or transmit an obscene message or indecent or profane words to a person.
INTIMIDATION- By Indiana statute IC 35-45-2-1, a person who communicates a threat to another person, with the intent: that the other person engage in conduct against the other person's will; that the other person be placed in fear of retaliation.
