Our service area includes Banks, Barrow and Jackson counties. If you do NOT have proof that you live in one of these counties, we cannot schedule an appointment. We will accept a driver's license or utility bill as proof.

Check in time is 8:45 AM — 9:00 AM. We do NOT accept late patients.

Pick up time is between 4 PM — 5 PM. You will be given a pick up time at check-in on the day of your appointment.
