Application: Lake County Business Social Distancing Protocol (general businesses, retail, government, nonprofits)
Lake County Public Health Agency (LCPHA) is working on reviewing proposed business protocols under the amended “Safer at Home” Public Health order. In order for your business operations to be evaluated, your business needs to:

1. Fill out all sections of this social distancing protocol.
2. Document any additional measures not included in the social distancing protocol shown here.
3. Email any additional documentation to environmentalhealth@co.lake.co.us.

Once the protocol & documentation is received, it will be reviewed by LCPHA. A determination will be made about ability to open and when (following guidelines set by Governor).

Upon LCPHA authorization, your business will be provided a letter of authorization within agreed upon parameters that will allow you to operate under "Safer-At-Home."
