In addition to filling this form, your PhD (or academic advisor for Master's/Undergraduate students) must send a letter of recommendation explaining: 
  1. The suitability of the Middleware program material to the student’s research area;
  2. Ways this particular student would benefit from attendance at the conference;
  3. The strengths and potential contributions of the student; and
  4. Why the trip cannot be funded from the advisor’s research grants and/or unrestricted discretionary funds.
