Broomfield FISH Volunteer Interest Form
Thank you for your interest in volunteering at FISH! We could not accomplish our mission of feeding families and fueling hope without YOUR help. A few things to note about volunteering:
If you are 15 or younger, you need to be accompanied by an adult.
You may complete your official volunteer application when you come in to FISH.
All volunteers will be required to sign a waiver. Volunteers who are 18 years or older must provide their social security number and birthdate for a background check. Those under 18 will need a signed parent permission form.
We are not accepting court appointed community service at this time.

Previous Work or Volunteer experience:
Volunteer Opportunities (check all that interest you)
Weekly Marketplace Volunteer (Half day each week commitment) Help clients through the self-shop Marketplace process.
Marketplace Substitute. Same as above, but on an as-needed basis.
Weekly Warehouse Volunteer (Half day each week commitment) Help sort donations and stock shelves.
Warehouse Substitute. Same as above, but on an as-needed basis.
Weekly Client Services Volunteer (Half day each week commitment) Greet our neighbors in need and assess how FISH can be of assistance. Answer phones and enter data into computers.
Client Services Substitute. Same as above, but on an as-needed basis.
Spanish Interpreter for clients.
Short Term Projects (notified when needed)
Driver to pick up donations from restaurants--no special license needed. (must be 25 or older)
Ride along to help load and unload truck (must be 18 or older)
Write the days and times you are available to volunteer. FISH is open Monday-Friday 9am-5pm with shifts available in the morning and afternoon.
Never submit passwords through Google Forms.
