Thanks for using our facilities! Please understand the following rules apply:

1) Treat all facilities with care, ensuring all persons respect the facilities
2) This application must be approved by the ASB cabinet, Athletic Director and Assistant Principal before activity begins
3) The advisor/coach will be responsible for supervising set-up, activity, clean up, equipment lock-up, and return
4) Organization/person will be charged for damaged, lost, or stolen furniture/equipment
5) Custodians are needed any time bathrooms will be opened for use
6) Please submit a separate form to April Stover if you have a specific room setup/layout that you need
