I understand that student enrollment is for entire 2024-25 dance year, as noted in the Contract & Handbook, which we have read together. Any changes or cancellation require 30 day written notice. We have read & agree to policies/procedures of Ballet Conservatory & payment terms in the Pre-Pro Schedule & Fees, both visible in the link above.

We will read, print, sign and submit a paper copy of Pre-Pro Contract in the link above.

CONSENT AGREEMENTS *

1) HEALTH & LIABILITY I certify participant (my child or myself) is in good health, capable of participating in all Ballet Conservatory of Asheville activities and classes. I recognize that participation in Ballet Conservatory classes, events, and activities may expose participant to the risk of injury or harm. I give consent for participation in such classes, events, and activities –including transportation to/from the same– and assume all risks and hazards, and hereby waive, release, absolve, and agree to hold harmless Ballet Conservatory of Asheville, its instructors, agents and employees, from all liability for personal injury, illness or property damage occurring on or off the Ballet Conservatory of Asheville premises, whether or not caused by the negligence of Ballet Conservatory of Asheville, its instructors, agents or employees. I understand dance instruction involves kinetic corrections to the body that may involve hands-on physical adjustments to the student as part of regular class work and rehearsals. If your family has recently been exposed, tested positive for COVID-19 or any contagious illness, or is potentially contagious please do not put other dancers at risk by coming into the studio. Notify BCA so we may plan together, and notify anyone else who may have been exposed. __________ 2) MEDIA Ballet Conservatory of Asheville is granted permission to use participant’s likeness in advertisements, news releases & on website.
